The UHNBC Pain Clinic is now accepting new referrals. The average wait time is approximately one year, with shorter wait times for priority referrals. We have developed a new pain service referral form and patient pain questionnaire. Both forms are available in MOIS, AIHS or can be ordered through Northern Health Document Source (document.source@northernhealth.ca or 250-649-7550, document #11-000-5172 and #11-129-5002).
Please ensure the referral form is completely filled out with the appropriate information attached. As well, the patient pain questionnaire must be filled out by the patient and returned with the referral form as one complete package. All incomplete referrals will be returned. Referrals require a complete history of the pain complaint, attempted management, any relevant investigations (X-rays, scans), any previous consultations or surgery, as well as any relevant medical history and medications.
