The BC cardiac catheterization referral form has been updated and is now available for inpatients and outpatients. On the second page of the referral form, there are details regarding the inpatient transfer process through the NH Cardiac Triage Coordinator. The TIMI score and GRACE score are also included on the second page of the referral form for the physician consult to assist with risk stratification. You can order the form through Northern Health Document Source (document # CARDIAC-001).
A new outpatient cardiac catheterization preparation booklet and inpatient cardiac catheterization preparation booklet for NH patients are now available to ensure patients are prepared for cardiac procedures and for going home. These booklets can be ordered through Northern Health Document Source (outpatient prep document # 10-005-6005, inpatient prep document # 10-005-6002).
