Updated Guideline for Colorectal Screening, Surveillance and Recall and Updated EGD and Colonoscopy Referral Form

Northern Health has released a memo with the updated the Guideline for Colorectal Screening, Surveillance and Recall, to provide further guidance around follow up.

This Guideline was developed by primary care providers and colonoscopists:
1. to provide evidence-based criteria for referral for colorectal screening/surveillance; and,
2. to inform providers of the recommended timelines for screening and surveillance follow-up for both average risk and increased risk patients.
Newly Revised: To provide guidance around the responsibility to provide follow up for asymptomatic patients.

Northern Health has also updated the EGD (Esophagogastroduodenoscopy) and Colonoscopy Referral Form.

Newly Revised: The form has been revised to add BMI, and section 4 on page 1 was adjusted to accommodate more space in the clinical history section for documentation.

We are asking that this form be completed by all NH Primary Care Providers and Internal Medicine Specialists when referring a patient to an endoscopist for an EGD or Colonoscopy.
