How to refer to the Regional Chronic Pain Clinic
Interventional and Interdisciplinary Pain program
General Practitioners and Nurse Practitioners can refer patients to the UHNBC Regional Chronic Pain Clinic for ongoing interventional care, access to our multidisciplinary team, and participation in educational programs. To ensure timely processing, please follow these guidelines:
- Complete the pain service referral form in full, including:
- The patient questionnaire
- All relevant medical history
- Any recent imaging reports
- If the patient has not had recent imaging, please arrange for this to be completed prior to submitting the referral.
- Incomplete referrals will be returned to the referring provider for completion.
Direct to Procedure Referral program
The Direct to Procedure program is designed to streamline referrals for one-time injection procedures at the UHNBC Regional Chronic Pain Clinic.
Program highlights
- Referrals are reviewed and scheduled with the goal of completing the injection within 8 weeks.
- This is a single-visit model: patients will follow up with the referring specialist only.
- The pain physician will not provide follow-up care unless specifically requested due to complications.

This referral pathway is intended for use by specialists only, including:
- Neurologists
- Neurosurgeons
- Orthopedic Surgeons
- Rheumatologists
