A memo has been released to all NH Primary Care Providers and Internal Medicine Specialists with information on the new EGD (esophagogastroduodenoscopy) and colonoscopy referral form (for viewing only) and the new Colorectal Screening and Surveillance Guide. Please refer to the memo for instructions on how to order the referral form and the guide.
The form is to be completed by all NH Primary Care Providers and Internal Medicine Specialists when referring a patient to an endoscopist for an EDG or colonoscopy and has been uploaded to all instances of community MOIS across NH.
The guide was developed to: provide evidence-based criteria for referral for colorectal screening and surveillance; and to inform providers of the recommended timelines for screening and surveillance and follow up for both average risk and increased risk patients for colonoscopy. A poster which illustrates the guide was developed for office use.
