The Mental Health & Substance Use (MHSU) regional team would like to circulate an update on the upcoming changes with the Mental Health Act: specifically, a change from Form 4 (Medical Certificate – Involuntary Admission) to Form 4.1 (First Medical Certificate) and Form 4.2 (Second Medical Certificate).
Key dates:
January 15, 2024 – Form 4 will no longer be in circulation; internal transition to Form 4.1 and 4.2. Remove all blank copies of Form 4 from units and replace with Form 4.1 and Form 4.2
January 31, 2024 – Final day Form 4 will be legally valid
February 1, 2024 – Form 4.1 and 4.2 will be legally required for involuntary admissions under the Mental Health Act
