New requirement to report all serious adverse drug reactions and medical device incidents

Starting December 16, 2019, all Canadian hospitals are legally required to report adverse drug reactions (ADRs) and medical device incidents (MDIs) to Health Canada within 30 days of the event being documented in the hospital.

If you’re the first person to discover an ADR or MDI, report it using the BC Patient Safety Learning System (PSLS). PSLS will review and forward ADR and MDI reports to Health Canada on behalf of all BC health authorities.

While this legal requirement applies only to serious ADRs and MDIs, all health care providers in all settings are encouraged to report all ADRs and medical device problems using PSLS.
