The Northern Health Kidney Care Project is responsible for the delivery of kidney care across the region. The project has put together an update in the form of an infographic on their accomplishments over the last year, as well as their future directions.
Their strategy is to provide better access to care for patients that is aligned with best practices. The project provides patients with advanced kidney disease with access to virtual team-based kidney care and is focused on education and building relationships with smaller and more remote communities.
